Direct Pay Model

In order to provide excellence in care, we adopt the direct pay model and do not accept insurance. Credit cards, HSA/FSA are accepted. This allows us to keep fees reasonable and minimize the complexity of billing to focus the care on you!

• Initial consultation fee: $160
• Establishing plan of care after initial consultation: $160
• Comprehensive follow up: $120
• Maintenance follow up: $80
• Bio-identical Hormone Replacement Pellet Pricing:
• Females (every 3-4 months) $350
• Males (every 5-6 months) $700
• GeneSight Cheek Swab: $40
• Call for additional cost of the test, since it is based on an income sliding scale.

All appointments are available either in person or telemedicine/Zoom

We offer a 10% discount off every office visit for active/retired military or first responders, with proof of ID. We thank you for your service!
